Care Transitions is a Medicare Quality Improvement program under the Centers for Medicare & Medicaid Services. Its goal is to reduce the rate of avoidable re-hospitalizations by improving the transition of patients across care settings.

AHRQ Medical Reconciliation QIO Learning Network Collaborative Learning Session 2,
September 8, 2010 – 8:30 a.m. to 3:30 p.m. CT. Save-the-Date. If your facility is a member of this free accelerated improvement collaborative, please save September 8 on your calendar for Learning Session 2. This is the last learning session in the collaborative, followed by four support calls between now and the end of October, 2010, when the Collaborative ends. More details will be posted soon. New Home Health Quality Improvement National Campaign Event—CNE and BPIP for October
