Are you eligible to participate?

Determine your eligibility to particpate in the Care Transtions Project by reviewing the map or entering your organization's Medicare Provider Number.

Only health care providers in the Lower Rio Grande Valley cities of Brownsville, Harlingen and Weslaco are invited to participate in this innovative health care improvement project.

Do I need to complete the Participation Agreement?

These providers are asked to complete a participation agreement:

  • Hospitals
  • Inpatient Rehab Facilities
  • LTACH Facilities
  • Home Health Agencies
  • Skilled Nursing Facilities

These other community providers and stakeholders are encouraged to participate in the project and register to attend conferences and other events but are not required to complete a participation agreement:

  • Physicians
  • Dialysis Facilities
  • Hospices
  • Clinics
  • Other Community Health Stakeholders
